After looking more closely, your notify request is malformed:
> Messages-Waiting: 10.
> Voicemail: 10/20.
These must be in message body and not headers and you must set the Content-Length appropriately.
Also, set the From and Contact headers values to the ip adress of the sending host ($srchost variable in sipsak template.)
